Intake Forms can be viewed by admin on the client record. Depending on the Intake Form’s configuration, a list of completed Intake Forms may also be listed on the Dashboard as soon as the client has completed them. This can then prompt you to click on the Dashboard item and allow you to view the completed Intake Form on the client’s record.
Viewing a completed Intake Form on the client record
Steps
Navigate to the Clients menu
Search for the relevant client
Select Client Profile
Click on the Notes tab
Click View Intake Form

If the Intake Form has been configured to raise an Action Requirement on the client record once client completes an Intake Form, then a list of completed Intake Forms will be listed on the Dashboard as soon as the client has completed them. This can then prompt you to click on the Dashboard item and allow you to view the completed Intake Form on the client’s record.
